Radiation and Soul Food in Rome. Also in Rome you should hang out in the Pigneto area where there are the best places for shows: Fanfulla, Dal Verme, 30 Formiche. These are bars/clubs.
Milano: Solo Vinili e Libri, Psycho, Metropolis (not cheap), if you're interested in jazz, musique concrete, italian prog/jazz, library music and all that stuff you should get an appointment with the guys at SoundOhm.com. They have a space where you can go but it's not a store but if you write to them they'll let you in their storage room.
Where else are you going? You should go to Bologna, it's great. Record stores: Disco D'Oro. There are others but not really good.
The south is supposed to be a blast but I'm not familiar. Go to Naples if you wanna see serious Italy like in the movies. Or like small towns in the far South like in the regions Calabria, Basilicata, Puglia, Sicily. You get places that still look like a Vittorio De Sica movie.
August is a shitty time for shows because it's really hot and clubs are closed, but I'll try to remember to keep this thread updated for open air music things.
